The Form 680 you are creating is not certified.
Benefits of Using Certified Form 680s
An electronically signed and certified form 680
is available and accessible to any authorized
health-care provider, school, or licensed child-care
provider enrolled in Florida SHOTS. With just a few
quick changes to your personnel list, your office
can create and certify (e-sign) electronic 680s for your
patients. The electronic signature means parents
no longer need to wait while a provider signs the
form. And if a child is missing their 680 form,
users can check Florida SHOTS to see if there is
an electronically signed and certified form available
from an authorized Florida SHOTS user.

Once created, the historic record is kept in
Florida SHOTS. It contains all of the regular
shot record information as well as a box in the
lower right corner that shows the certifying (e-signing)
office information.

Certified forms will be accessible directly to
schools, licensed child-care centers, and other
medical providers that are authorized Florida
SHOTS users.

At your option, you can allow parents to access Certified
forms from the Florida SHOTS website from their own
computer.

You can search for previously certified forms created
by any authorized Florida SHOTS participant.

You can search for all forms certified by your
organization in a specific time.
How to Set Preferences to Produce
Certified 680s
- Your local account administrator must
update the personnel lists on the personnel
maintenance page. This requires two steps:
- Set up permissions for individuals whose
names will be used to electronically sign
the new 680 forms ("Certify Form 680").
- Set up permissions for users who will
need access to issue the certified forms
("Create Certified Form 680").

Once your office's preferences are set, create a
new certified 680 every time you update
an immunization.
