Thank you for your interest in serving your community by providing COVID-19 vaccinations. The online enrollment process to gain access to Florida SHOTS is quick and easy and will continue on the following pages. Be sure to check the "COVID-19 Enrollment" box at the end to note your organization's interest in participating in the COVID-19 vaccination effort.
Note: If your organization already has access to, or communicates with, Florida SHOTS, please login as normal and proceed to "COVID-19 Enrollment" in the main system menu. Keep in mind that many organizations communicate with Florida SHOTS directly via their EMR/EHR system. If you're not sure, please contact the Florida SHOTS Helpdesk prior to enrolling for access.
COVID-19 Vaccine Enrollment Process Overview:
Once your organization gains access to Florida SHOTS, click on the “COVID-19 Enrollment” link on the main menu. The CDC COVID-19 Vaccination Program Provider Agreement
must be completed for the organization (Section A) and each clinic location (Section B). Please review the help text on that page for detailed instructions on completing each section.
Enrollment with the Florida VFC/VFA Program will be required for each clinic location for which vaccines will be ordered and received. After completion of CDC COVID-19 Vaccination Program Provider Agreement
Section A, VFC staff will contact providers to begin the VFC enrollment process as needed. Section A must be complete prior to the completion of Section B for a given location.
