Follow these easy steps to order printed copies of the English/Spanish Recommended Childhood Immunization Schedule Poster or refills of the Childhood Immunization Schedule pads for the poster.
- Print out the order form and follow the instructions on the form.
- Order using Stock Number 5731-917-0150-3 (for poster) or 5731-916-0150-9 (for immunization schedule refill pads).
- Indicate quantity. (Refill pads are provided in packs of 10. Posters are provided in packs of 50. If you’d rather have fewer than 50 posters, please email your order directly to with the exact quantity requested instead of faxing the form to the warehouse.)

- Include your check for shipping charges with your order so that your order will not be delayed.
