To have your child’s immunization records included in Florida SHOTS, your child’s doctor must enroll.

The enrollment process is free, fast and easy for your child’s physician, and ensures that your child’s immunization records are available whenever and wherever your doctors need them, no matter what.

You have two easy ways to ask your doctor to enroll in Florida SHOTS.

  1. Download this flyer to take enrollment and benefit information about Florida SHOTS with you to your next doctor’s appointment.
  2. Please email your doctor's information to , including: doctor's name, practice or organization name, city, county and telephone number (with area code). We will have a Florida SHOTS regional coordinator follow up with them to make sure they are enrolled in and using the registry.
